Adolescent / Adult Circumcisions
Adult circumcision is most often performed for true medical necessity, at least in Western societies. It may also be chosen for cultural, religious or purely personal/aesthetic preference. Dr. Eiley performs circumcisions on adolescents and adults of ALL ages in the comforts of a state-of-the-art office facility under full local anaesthesia.
Although reported complications are few, they can be significant, and hence expertise is necessary. Unlike newborns, the adult will have already maximized his penile growth after puberty. Thus, it becomes more important to assure that adequate shaft skin is protected when doing a circumcision.
There are many variations of available techniques and, hence, most urologists and surgeons will incorporate their own preferences when performing circumcisions on adults.
Why should I have a circumcision?
Some reasons to undergo a circumcision as a teenager or adult include the following:
- Personal preference (including cosmetic, hygienic)
- Religious customs and beliefs
- Partner preference. Surveys indicate that many women prefer a circumcised vs. an uncircumcised penis
- Medical needs including:
- Phimosis (inability to retract the foreskin)
- Paraphimosis (inability to return a retracted foreskin to its original position)
- Balanitis (swelling of the foreskin)
- Balanoposthitis (inflammation of the tip and foreskin of the penis)
- Prevention of certain STIs (Sexually Transmitted Infections), penile cancer, and some infections in female sexual partners
Pre-operation Instructions
- Shave the entire pubic area and base of the penis. This can be done with an electric clipper or a blade razor. This is done to reduce the infection rate. The hair will grow back with time
- You may eat a light meal and drink before the procedure (there is no need to fast)
- Consider bringing headphones/earbuds if you would like to listen to your own music, podcast, etc.
- Please come accompanied by someone to drive you home. You should not drive a car immediately after the surgery
- If you are taking blood thinner medications, they should be stopped beforehand as per the doctor’s instructions
The Surgery
- You will be lying on your back
- A nurse assistant will be present throughout
- A complete sterile technique will be employed
- A disinfectant is applied to the skin and sterile disposable drapes are placed around the penis. Completely sterilized instruments from an autoclave are utilized
- Local anesthesia is performed to the penis only. Full freezing will be confirmed prior to proceeding.
- The technique used is a “Sleeve Resection Procedure.”
- variations of the technique individualized to patient’s preference and anatomy
- Not identical for everyone (please see photos with brief descriptions below)
- Hemostasis is assured (i.e. no bleeding) before wound closure
- Fine stitches (subcuticular +/- interrupted) and/or skin glue are used for skin closure
- The frenulum can be preserved or released (frenuloplasty) at the same setting depending on patient preference and/or symptoms
- An antibiotic ointment and a gauze dressing is applied
- The entire procedure takes approximately 40 minutes
Technique Variations
Cosmetic and patient preference taken into account; high vs low, tight vs loose. Dr. Eiley is most often able to customize the amount of foreskin removed (high – low incision line) and the resulting tightness of the shaft (loose – tight). Frenular lengthening The Frenulum is the linear band of tissue on the ventrum (under surface) of the penis. It is attached to the glans to the shaft of the penis. Frenulum Breve is a condition where the frenulum is short, or tight, and causes bending or tenting of the erect or flaccid (limp) penis. It can cause pain with sexual activity thereby limiting the sexual act. In addition, it can tear and bleed considerably. A tight frenulum has also been associated with premature ejaculation in some men. The frenulum can be released during a circumcision, or as a stand-alone procedure. The healing is usually complete by 3 weeks. When Dr. Eiley performs a frenuloplasty, he ensures that the scar line is linear, in the midline and in the exact same place where the frenulum was. |
Like the previous style, this one removes a lot of the inner foreskin but removes more shaft skin overall resulting in a much tighter look and will as a result keep the glans permanently exposed including the corona.

When Dr. Eiley performs a frenuloplasty, he ensures that the scar line is linear, in the midline and in the exact same place where the frenulum was.
This style removes most of the inner foreskin while removing the least amount of foreskin available. The nature of this style could still lead to some bunching of skin behind the corona (ridge) and potential partial coverage of the glans (penile head) when flaccid. This style does remove a lot of the sensitive inner foreskin, but keeps the scar close to the head of the penis.
Like the previous style, this one removes a lot of the inner foreskin but removes more shaft skin overall resulting in a much tighter look and will as a result keep the glans permanently exposed including the corona.
This style like low and loose can leave a fair bit of extra skin which will cause some bunching behind the corona. This style though keeps a lot more of the sensitive inner foreskin and places the scar further down the shaft.
This style keeps more inner foreskin thus creating a tighter style keeping the glans and corona fully exposed even when flaccid
Post operation information
- You can leave the office immediately after the procedure
- You should rest and avoid heavy physical activity for 2 days after the procedure
- You can remove the dressing 2 days later if it is still on. If it is stuck to the skin, apply some water to the dressing and retry
- Antibiotic ointment should be applied to the suture line twice/day for 3 weeks
- Showers are fine the following day but do no soak the penis (bathing or swimming) for 3 weeks
- You can wear either tight or loose underwear; whichever feels better
- Avoid sexual activity for at least 1 month
- If you get an erection, it will not cause damage
- Pain medications may be needed for the first 2-3 days
- Rarely, antibiotics will be prescribed by the doctor
- Bruising and swelling is normal and may take 2 weeks to resolve
- Any stitches or glue placed will melt away on their own. Nothing has to be removed
A follow-up appointment will be made in about 4-6 weeks. You can contact the office and be seen earlier if necessary.
Complications
- The risks of having a circumcision are very low
- If you experience active bleeding in the first day or so, apply pressure by squeezing around bandage/suture line for 5-10 minutes. You can apply an ice pack as well
- The infection rate is low. If there are fever, chills, penile redness, wound drainage (i.e. pus) or concerns, please call the office immediately
- Suture disruption (when part of the wound opens) is very rare. If this occurs, call the office immediately
- Pain is usually mild to moderate in the first 3 days. Acetaminophen can be used. The Doctor will prescribe a stronger pain killer if necessary
- Having a circumcision in the long term will NOT cause:
- Urinary problems
- Erectile dysfunction
- Decreased sensation of the glans (penile head)
